Our Responsibilities

Notification of a Breach

If a breach of unsecured protected health information occurs that affects your privacy, we will notify you as required by applicable federal and state law and provide information regarding the steps being taken to address the situation.

Questions or Complaints

If you believe your privacy rights have been violated or you have questions about our privacy practices, please contact our office. You may also file a complaint with the U.S. Department of Health and Human Services. Filing a complaint will not affect your care or services with our practice.
