Complete your paperwork before your first appointment to help make your visit as smooth and efficient as possible.
Getting Started Is Easy
We ask all new clients to complete their intake paperwork before their first appointment whenever possible. Completing your forms ahead of time allows your provider to spend more time focusing on your care during your first visit.
Once you request your forms, our office will send your new client paperwork to the email address you provide.
1️⃣ Request Your Forms
Click the Request New Client Forms button or contact our office. We’ll send your intake paperwork to the email address you provide.
2️⃣ Complete Your Paperwork
Complete all forms before your appointment. If you have questions while filling them out, our staff is happy to assist you.
3️⃣ Bring Required Information
Please have your government-issued photo ID, insurance card (if applicable), and any referral or authorization required by your insurance plan.
What to Bring to Your First Appointment
To help your appointment begin on time, please have the following available:
- Government-issued photo ID
- Current insurance card (if using insurance)
- Referral or authorization, if required by your insurance
- A list of current medications (if applicable)
- Completed intake paperwork
What Happens Next?
- Request your new client forms.
- Our office will email your intake paperwork.
- Complete and return the forms before your appointment whenever possible.
- Our office will review your paperwork and verify your insurance benefits, if applicable.
- Arrive approximately 15 minutes before your scheduled appointment.
Insurance & Payment
If you plan to use insurance, please provide your current insurance information when requesting your appointment. Our staff will verify your benefits as a courtesy whenever possible.
Copayments, deductibles, and any applicable patient responsibility are due at the time of your visit.
